Job Summary

The CNA/NA/HHA/DSP position serves as caregiver to our individual during the scheduled work period in Home Care settings. They provide individual-centered care and daily living assistance under the supervision of a registered nurse (RN). They have knowledge of and deliver age-appropriate care related to the physical and psychological needs of the individual as per the care plan. Considered a member of our care team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use. Aids with basic health care needs, including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing cognitive support and other personal care to assigned individuals. Assists the individual in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment, and provides a range of motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the individuals. Assists individuals with meals and snacks, provides water and supplements as needed.

The CNA/NA/HHA/DSP documents individual interactions as needed and the outcomes related to individual care, behavior, activity, and diet. Communicates the individual’s changing condition and care-related concerns/responses to the care coordinator. Provides housekeeping and laundry services to assist individuals with keeping living areas clean and operational. Provides and documents wellness and recreational programs and encourages individuals to participate.

Qualifications

  • Minimum age of 18
  • High school graduate or general equivalency diploma (GED) preferred
  • Prior clinical or nursing assistant experience in person-centered care preferred
  • Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or a state-certified CNA program preferred
  • If the employee does not have prior training, the employee will be required to enroll in a training program provided by the facility within four months of hire
  • Satisfactorily completes yearly department-specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours of continuing education is required annually. Maintains all department-specific required certifications
  • When applicable, completes state-required training
  • Depending on the individual and required responsibilities, this role may be classified as a Driver requiring a motor vehicle report and proof of a valid driver’s license. Also, must be medically qualified to perform the essential driving functions of this position as per our Driving Policy, per the leader’s request
